Telepsychiatry and telepsychology services allow us to bridge the geographic barriers to care that many patients experience. 

 

Telehealth — sometimes called telemedicine — is the use of telecommunication technologies to provide care when you and your healthcare professional are not in the same place at the same time. If you have a “smart phone” or a device with internet access, you already have everything you need to get CounterPoint services through telehealth. CounterPoint telehealth allows you to talk “live” with your healthcare provider and send and receive important messages.  

CounterPoint telehealth services are available for adults and older adults residing in the mid-Atlantic and other states. Our telehealth services include:

  • Comprehensive neuro-cognitive and mood evaluations
  • Individual supportive therapy
  • Support in the discharge planning process
  • Family consultation and therapy
  • Development of behavioral management plans
  • Initial & follow up psychiatry evaluations
  • Psychotropic medication management
  • Gradual dose reductions

Whether you live at home or in the community, in a post-acute care setting like senior living or a nursing home, or some other living environment, we are here to help. Our unique approach to telepsychology and telepsychiatry allows us to help in three distinct ways.

CounterPoint center-based telehealth services integrate in-person psychiatric and psychological visits with telehealth visits for residents in nursing homes and senior living communities. These services allow us to provide care 24 hours a day, seven days a week, supporting facility staff and families. Residents can receive in-person visits and/or telehealth visits.  This allows us to stay connected and improve facility quality measures and health outcomes.

CounterPoint “transitions in care” telehealth services support patients discharging from nursing homes and hospitals.  One of the gaps in the healthcare system that contributes to unnecessary re-hospitalizations is a lack of immediate mental health follow-up after discharge. Our program solves this problem, and we have demonstrated that the telehealth “transitions in care” services reduce hospitalizations and adverse events.

CounterPoint community-based telehealth services provide virtual psychological and psychiatric evaluation and treatment to patients living in the community.  These services are particularly important for patients who do not have easy access or transportation to a mental health provider.
